What are your policies regarding confidentiality?
All information you provide is confidential and cannot be revealed without your permission. With a signed written release of information, I will be happy to work in conjunction with your medical treatment team. There are a few exceptions in which I am mandated by the law to breach confidentiality. This is explained thoroughly in the Resources section.
What is your cancellation policy?
If it's necessary to cancel an appointment, you must provide 24 hours notice for cancellation prior to your scheduled appointment time. You will be charged the full fee for missed or cancelled appointment unless you have provided 24 hour advance notice. All voicemails are dated and timestamped.
Do you prescribe medication?
I do not prescribe medication. However, when appropriate, I will refer you to a prescribing provider for a medication evaluation. Additionally, I will be happy to work in conjunction with your medical treatment team.
Do you take insurance?
I am considered an out-of-network provider, which means that I am not contracted by insurance plans. However, many insurance plans do provide the opportunity to apply for reimbursement for therapy services received from out-of-network providers. Plans can vary widely, and you are responsible for determining what insurance benefits you receive and can obtain for reimbursement. In some cases, you can request a "single case exception", explaining why you are unable to find another provider with my same expertise in your area, and they may be able to cover the treatment. Please call your insurance company (phone number is on back of insurance card) prior to your first appointment and request further information regarding your out-of-network benefits.

- Is there a deductible?
- How many sessions per year does my plan cover?
- What information do I need to submit in order to receive out-of-network reimbursement?

How do I get to your office?
My office is conveniently located in the Kinnard Building in Nashville, TN. The address is 2200 21st Avenue South, Suite 252, Nashville, TN 37212. Click here for directions. The Kinnard Building is on the corner of 21st Avenue South and Blair Blvd., between Hillsboro Village and I-440. The building is across the street from the new Kroger Grocery store, and directly next to Hillsboro Hardware. The office is handicap accessible.
There is complimentary covered parking under the building with the entrance off Blair Blvd. If this lot is full, there is an additional lot that is accessed between the Kinnard building and the Hillsboro Hardware on 21st Avenue.
My office is on the 2nd floor, Suite 252. When you exit the elevator on the second floor, take a right out of the elevator, then immediately turn right to walk down a short hallway. The door is on the left and the waiting rooms is directly across.
How do I make a payment?
I accept checks and credit cards. I am considered an out-of-network provider, and do not accept insurance plans. However, I am happy to provide you with an invoice to submit to your insurance company for reimbursement.
